Patient Safety Incident Response Framework (PSIRF)
All NHS Trusts and providers are preparing for the introduction of a new Patient Safety Incident Response Framework (PSIRF).
One of our key priorities is to improve care, safety and quality. We recognise the significant impact that patient safety incidents have on patients, service users, their families and carers.
From Autumn 2023, our Trust will start to use the PSIRF to identify learning and make improvements following a patient safety incident.
PSIRF is a major step towards establishing a safety management system across the whole of the NHS.
The PSIRF supports the development and maintenance of an effective patient safety incident response system that brings together four key aims:
- Compassionate engagement and involvement of those affected by patient safety incidents
- Application of a range of system-based approaches to learning from patient safety incidents
- Considered and proportionate responses to patient safety incidents
- Supportive oversight focused on strengthening response system functioning and improvement
