Intensive home based treatment team

The two main roles of the intensive home based treatment team are to assess people experiencing a mental health crisis who may need to be referred to an inpatient unit, and to care for people in their own homes as an alternative to admission to a mental health unit.

The assessment function will see people referred to the team who are experiencing a crisis in their mental health. They may be feeling unsafe and having thoughts to harm themselves, or they may have an existing mental health problem and feel that they are relapsing.
The team will see people within 4 hours of receiving the referral, and also follow up any person discharged from an inpatient unit within 7 days.

The home based treatment function intensively nurse people in their own homes as an alternative to admission to a mental health unit. The team work closely with family and carers in order to provide nursing care. They are able to see people on a daily basis while they are unwell and work to enable recovery.
