Chronic Obstructive Pulmonary Disease (COPD) service
The community COPD service provides a specialist service for patients, carers and families of individuals diagnosed with a chronic obstructive disease such as asthma, bronchietasis, lung cancer etc.
The service aims to help facilitate the best possible quality of life for patients, offering self-management techniques,medicines optimisation and facilitation into pulmonary rehabilitation. The service is made up of a specialist team which supports patients from pre-diagnosis, complex cases through to palliative care and end of life.
We aim to facilitate prevention of admissions and reduce bed days in hospital. We work alongside national and local quality standards based on evidence based practice and also provide a community based oxygen service alongside the department of health guidelines.
