Diabetes specialist nursing service

The diabetes specialist nursing service is a nurse-led service for adults with a confirmed diagnosis of diabetes. The service provides support, education and information about diabetes to patients, carers and families and other health care professionals. The service provides face to face consultation, telephone and online consultations and education programmes in a variety of health care settings.

Why would someone choose the service

  • The service is provided by a team of highly skilled nurses who work in partnership with the individual to achieve improved health outcomes
  • We are a core member of the Barnsley Local Diabetes Service Advisory Group which has representation from all those involved in the delivery of care to individuals with diabetes, including patient representatives, ensuring that service developments are reflective of the needs of its patient group
