Care home liaison team

The care homes liaison team – which includes nurses, a dietitian, and access to a consultant psychiatrist – provides assessment and support to people with mental health problems who are permanent residents in care homes in Calderdale.

We work with individuals as well as supporting care staff in the care of people experiencing mental health problems, though most work involves people experiencing dementia.

We work with colleagues in social services and with the QUEST team (a team providing support to care homes) in the assessment and care of people in care homes.
