Community mental health team older people’s service (East, West and older people’s service)
The community mental health team for older people provides assessment, treatment, care planning and interventions for service users who are over 65 so they can be supported to manage their mental health conditions, while promoting independence and encouraging choice.
We also conduct memory/dementia assessments for service users aged 18+.
We work with partnership organisations such as Alzheimer’s Society to provide tailored and supportive mental health care.
The intensive support team based at The Dales, Calderdale Royal Hospital, carries out work with service users on Saturdays and Sundays on a needs-led basis.

- Following a referral to the service, a member of the team will visit you at your home or an alternative agreed venue.
- You will be allocated a care coordinator who will be your main point of contact and will help you to identify your needs, treatment and interventions that may be right for you.
- Our staff will help you work at your own pace and always respect your wishes.
- Carer support to help relieve stress and explore their concerns is available through a carer’s assessment.
