Learning disability nursing team (adults)

The learning disability nursing team provides specialist support, advice and information for adults with a learning disability in the Calderdale area. Our role also involves supporting family members and carers.

We provide personalised care for all of our service users who receive individually tailored health plans. We actively promote the use of health action plans so that they are recognised and used by all services that might support an individual during their care.

Whilst supporting service users with their learning disability we also ensure that their physical health is given the same level of attention and care. We provide access to health screening programmes and work with GPs to develop annual health checks. We also make sure the mental health needs of service users are catered for and conduct regular mental capacity and best interest assessments.

As part of our commitment to working alongside other services and professionals to provide the best care for people in Calderdale we work with mainstream services to enhance their skills and knowledge to meet the health needs of people with learning disabilities. This helps us to signpost service users to other suitable local services where appropriate.
