Community mental health team (CMHT) 3 (Horbury)
Community mental health team 3 is one of four community mental health teams for working age adults in the Wakefield area. The team aims to meet the needs of, and improve outcomes for individuals with severe and complex mental health problems and their carers.
We cover the west of the Wakefield district and offer a service to people aged between 18 and 65, focussing on the needs of people with psychosis and affective disorders such as depression.
The team offers assessment, care management and a range of evidence based treatments to improve mental wellbeing. We are committed to a person centred recovery.
Services are provided through the Care Programme Approach which is a particular way of accessing, planning and reviewing someone’s mental health care needs. If individuals have social care needs, and are eligible under Fair Access to Care Services criteria, social care support is offered through personalised and self-directed support.
We work in partnership with all other areas of mental health and social care services in the area. Our aim is to enable people to live life to the full and improve outcomes for people by providing quality support to the individual and those close to them.
