If you are registered to a Kirklees GP and are experiencing a common mental health condition such as stress, depression or anxiety, and would like some help to manage it – you can now self-refer to the Kirklees IAPT team.

Complete the form below, or if you prefer complete our self-referral form – you can do this on your computer or print it off. Once you have filled it in, you can return it to the team by email, fax or post.

Don’t forget – if it works best for you, you can still go to your GP to ask them to refer you to the service too.
