Community mental health team (CMHT) 4 (Pontefract)
Community mental health team four is one of four community mental health teams helping working age adults in the Wakefield area. The team aims to meet the needs of, and improve outcomes for individuals with severe and complex mental health problems and their carers. The team is made up from a wide range of professionals based in communities across the Pontefract area of Wakefield.
Community mental health team four offers help to people aged between 18 and 65, focusing on the needs of people with non-psychotic presentations, for example personality disorders, severe depression or anxiety disorders.
The team offer assessment, care management and a range of evidence based interventions to improve mental wellbeing. We are committed to putting individuals at the centre of their recovery.
Services are provided using the Care Programme Approach, which is a particular way of assessing, planning and reviewing someone’s mental health care needs. Where individuals have social care needs and are eligible for help under the Fair Access to Care Services criteria, social care support is offered through personal, self-led support.
We work in partnership with all other areas of mental health and social care services in the area. Our aim is to enable people to live life to the full and improve the lives of people by providing quality support to the individual and those close to them.
