Welcome to our feedback centre.
As a Trust we provide mental health, community, learning disability and wellbeing services across Barnsley, Calderdale, Kirklees and Wakefield. We also provide specialist secure mental health (forensic) services for the whole of Yorkshire and Humber.

If you are using a computer and have a smartphone to hand, you can scan the QR code to access the Friends and Family Test survey.
To help you identify the service you wish to provide feedback for, our survey organises teams in the following way.
- Area: Barnsley, Calderdale, Doncaster, Kirklees, Leeds Sheffield, Wakefield and Wetherby
- Service: Child and Adolescent Mental Health Services (CAMHS), learning disability services, community mental health services, mental health inpatients, Forensics Inpatient and community physical health services. Our community physical health services include children’s physical health in Barnsley
- Team: Teams are listed alphabetically; some teams will provide you with a code to help you identity which team to choose from the list
