I now have the information to self-care and manage my condition

Heart failure and atrial fibrillation saw David lose interest in life and he spiralled into depression. David now has telehealth equipment in his house so that the care navigation service can monitor his health and wellbeing every day. He learnt about depression and now feels much more educated about how to self-care.
