Altogether Better

The Trust is delighted to be hosting a successful and ambitious approach to finding new ways to work with citizens which not only improves health outcomes for people but also makes life better for everybody involved in providing services.

The NHS Five Year Forward View asks us to consider the possible futures on offer and the challenges we face in the NHS. It argues that health services need to change and develop a more sustained relationship with patients, carers and citizens. There are few examples where citizens have made a significant change, through participation rather than consultation, to the way that general practice and wider health and social care services are designed and delivered.

The Altogether Better team has developed an evidence-based approach called Collaborative Practice which responds to this challenge. The team are working in 18 CCG areas around the country, in settings including nearly 100 GP practices as well as in specialist hospital services, A&E and residential care homes. The outcomes in general practice have been transformational and include better outcomes for patients and practices and a new sustainable business model for general practice.
