Connecting Care Wakefield District update

Or Trust is part of Connecting Care, an ambitious programme to transform care across Wakefield. It’s led by NHS Wakefield Clinical Commissioning Group and Wakefield Council.
The work to transform and integrate health and social care for local people is earning us national recognition, with high profile visitors on the patch keen to learn from how we’re using Connecting Care in practice.
We’ve also been given unqualified approval for our Better Care Fund plans for 2016/17. This money will be used to jointly plan and deliver local services.

Wakefield District has recently been asked by NHS England to work with them on setting up a new approach to commissioning and providing out-of-hospital primary and community care services. They’re calling this a ‘multispecialty community provider’ (MCP) contract. We will be one of just six areas in the country developing this approach which will be a blueprint for authorities across England. It builds on the excellent work done in the west of Wakefield under the Vanguard programme, and is a real tribute to partnership working across the whole of our district.
