The team were successful in securing the Top Team award, which recognised teams that have shown dedication to delivering their aims and objectives and achieved great results for their customers. Judges were looking for examples of teams who had demonstrated true team spirit and provided an improved service as a result of working together.

The team were also awarded the Champions of Change award, which was presented to teams or individuals who had shown an innovative way of finding solutions for their service. This award praised teams who had been positive in the face of change.

The community mental health team for older people is made up of staff from South West Yorkshire Partnership NHS Foundation Trust and Wakefield Council. The team provides community mental health care for people who are over 65 years old who are experiencing difficulties with their mental health.
