About the service
BREATHE are a multidisciplinary team providing specialist treatment and support, seven days per week, for patients with a diagnosed respiratory condition. The service offers patient centred care in clinics around Barnsley to make it easily accessible and, if required, will support patients in their own home to manage their condition.
Once you have been referred to the BREATHE service you will be contacted and invited to a clinic appointment or a home visit will be arranged if that is more appropriate. The BREATHE staff member will begin to assess your needs and talk a little more about what the service can offer. Also, they might suggest onward referral to support services such as pulmonary rehabilitation.
If you are unsure if you are appropriate for referral then please seek further information from your GP.

The service offers specialist respiratory support that is accessible to all. The multi-disciplinary team can answer any questions that you have regarding your condition and give you advice and guidance on the best management plan for you.
Staff you may meet
- Administrative staff provide essential support to doctors, nurses and other healthcare professionals. This can be in a variety of different settings, with administrators working as a receptionist in a clinic or a clerk on a ward. They may also be working closely with a consultant as a medical secretary.
- There are more than 60 different specialities that doctors work within the NHS. Each is unique but there are many characteristics which are common. Roles range from working in a hospital to being based in the community as a GP.
- Nurses who choose to specialise in the mental health branch of nursing work with GPs, psychiatrists, psychologists, and others, to help care for patients. Increasingly, care is given in the community, with mental health nurses visiting patients and their families at home, in residential centres, in prisons or in specialist clinics or units.
- Occupational therapy is the assessment and treatment of physical and psychiatric conditions using specific, purposeful activity to prevent disability and promote independent function in all aspects of daily life.
- Physiotherapists help people to improve their range of movement in order to promote health and well being. This can help people to live more independently.

Outcomes
The main aim of the service is to optimise treatment and educate patients to manage their own condition. Patients should be able to recognise any changes in their condition and when they need to seek additional support.
Referrals accepted from:
A & E, AHPs, Consultants, GP staff, GPs, Hospital staff, Other NHS services, Other Trust services, Single Point of Access team
Referral criteria:
BREATHE will accept referrals from healthcare professionals for patients that meet the inclusion criteria and can offer the following service:
- Early supported discharge and post discharge support (including provision of short-term nebulisers if required)
- Home oxygen assessment for stable patients N.B patients requiring oxygen assessment for hospital discharge should be referred to the inpatient oxygen team
- Ambulatory oxygen assessment
- Long term nebuliser assessment and review
- Complex case management
Exclusion criteria:
- Heart failure (unless requiring community oxygen assessment)
- Acute chest infections
- Pulmonary Rehabilitation
- Sleep disordered breathing
Patients with the following respiratory conditions will be seen by the team:
• COPD
• Bronchiectasis
• Cancer – EOL
• Pneumonia
• LRTI with diagnosed respiratory condition or requiring specialist respiratory input.
• Respiratory rehabilitation pathways (including COVID management and rehabilitation)
• Other chronic respiratory conditions
