About the service
The community rehabilitation team (CRT) are a team of occupational therapists and occupational therapy associate practitioners. We work with individuals aged between 18 and 65 who are under the care of a secondary care mental health team and provide specialist occupational therapy input.
We offer assessment and treatment in a person’s own home and community environment. We provide an intensive approach to support an individual’s functioning when the level of functional need cannot be met solely by their mental health team.
The length of time we will support someone varies on their individual needs and the support required, but it can last up to two years and can involve more than once daily contact.
The aim of the team is to develop an individual’s independence, so they are less reliant on services and more self-directed in managing their own mental health and recovery.
