(HPV) Vaccination consent form

The Human Papillomavirus (HPV) vaccine that protects against several types of cancer is being offered to your child at school.  Before completing the e-consent form please read the following information about the HPV vaccination programme to ensure that you are fully informed.   This information is available in several different languages.  Please discuss this with your son or daughter before completing the form.

Please only complete this form if your child is in Year 8 and attending a Barnsley school or they are being home-educated and live in Barnsley.

Information about the vaccinations will be entered into your child’s health records.

The Children’s immunisation team will commence this programme from the end of January 2021 and the second dose with be given in early 2022.

Any side effects following vaccination should be reported to the Immunisation team and/or your GP
