We want young people who use our services to receive the best care and treatment possible. To help us do this, we want to share information about the people who use our services with other health and care services that support you as well. We also want to have access to the information held by other health and care services too. This helps us join up care and prevent service users, children and young people having to provide the same information to each service providing them with care or support. its helpful if we can share information between services to get the best idea of what support can be most helpful for you.

We ask for a phone number so we can contact you if we need to help you or are worried if you are at risk from harm.

We understand that sharing a phone number is personal, and you may have good reasons for not wanting to be contacted by phone. We will only use your number to support your care and keep you safe, and only healthcare professionals who need it will be able to see it.

If you don’t want to give a phone number, we can look at other safe ways to stay in touch with you. However, sometimes this might mean it takes us a bit longer to get back to you

If we are unable to make contact with you and there are concerns about your safety, we will contact an adult to support you - this could be a parent, a schoolteacher or your GP.
