What does consent to sharing information mean?

We want young people who use our services to receive the best care and treatment possible. To help us do this, we want to share information about the people who use our services with other health and care services that support you as well. We also want to have access to the information held by other health and care services too. This helps us join up care and prevent service users, children and young people having to provide the same information to each service providing them with care or support. its helpful if we can share information between services to get the best idea of what support can be most helpful for you.
