Trust awarded SIRAN accreditation by Royal College of Psychiatrists
The Trust has been formally awarded the Safety Incident Response Accreditation Network (SIRAN) accreditation by the Royal College of Psychiatrists.

The SIRAN accreditation recognises organisations that demonstrate robust systems for reporting, reviewing and learning from safety incidents. Achieving this status reflects the Trust’s ongoing commitment to patient safety, transparent incident response and continuous quality improvement across mental health and community services.
The accreditation followed a comprehensive review of the Trust’s documentation and processes by the SIRAN committee. The panel assessed the quality of incident reporting practices, learning review processes, engagement and involvement of those affected, and evidence of local and organisational learning.
Prof Subha Thiyagesh, Chief Medical Officer at the Trust, said: “This is great news. This achievement clearly validates our culture and approach within our Trust and our commitment to a continuous focus on learning and patient safety. Thank you to everyone who contributed to this achievement.’’
The accreditation demonstrates that the Trust has effective governance arrangements in place to review incidents, identify learning and implement changes that reduce risk and improve outcomes for service users and staff.
The three year accreditation period provides a platform for continued development and further enhancement of incident response systems, ensuring that lessons learned translate into safer care and better experiences for people who use Trust services
